Waivers
Participants can access applicable trip waiver and emergency contact forms at:
https://buckiserv.osu.edu/secure/trip/trip_forms.aspx
The waiver forms do not take an actual signature - participants click the box at the end of each form to show agreement.
Registered participants should be able to see two documents:
- General Waiver, Release and Indemnification
- Emergency Contact Information

Participants who need to request to arrive late or depart early should complete the attached waiver form and return it to buck-i-serv@osu.edu .
